1. Wall test
Heels, glutes and upper back on the wall, feet 5cm out. Without poking the chin, bring the head back as far as it goes. Measure the gap from the back of the head to the wall, in cm. Lower is better.
2. Supine overhead reach
On your back, knees bent, low back pressed flat to the floor. Reach both arms overhead, elbows locked, thumbs up. Measure the gap from the back of the hands to the floor, in cm. If the low back arches off the floor, reset — that rep doesn't count.
3. Chin tuck hold
Lying down, nod first, then lift the head about 2cm off the floor while keeping the nod. Hold until the chin pokes forward or she shakes — record seconds. Under 20s is weak; 30–40s is a solid target.
4. Side profile photo
Same wall, same distance, same light, relaxed stance, eyes forward. Monthly. Purely a measurement aid — it makes head and shoulder position easier to compare across weeks than the tape measure alone.

What's actually going on
Three things stack. A constant anterior load pulls the upper back into flexion. Hours of looking down hold the head in front of the shoulders. And the shoulder blades sit tipped forward and downwardly rotated — which is exactly why she can't get her arms overhead. The blade has to tip back and rotate up for the arm to clear, and it can't if the upper back won't extend. Roughly 13–15° of thoracic extension is needed just to reach full shoulder flexion.
Why it's built this way
- Strengthen more than stretch. Stretching pec minor alone gives modest, short-lived change. Lower trap strengthening combined with pec stretching clearly beats stretching alone.
- Load through full range instead of stretching separately. Full-ROM resistance training improves range about as much as static stretching does, and adds strength at the same time. That's why her pressing has a deep stretch at the bottom rather than more pec stretching bolted on.
- Specific choices. Prone Y raise, high scapular retraction and prone external rotation at 90° produce the best lower-trap-to-upper-trap ratios in EMG testing. Serratus work is deliberate — when serratus is weak, upper trap compensates, which is part of the shoulders-up-and-forward look.
- Overhead pressing is earned. Pressing starts on a landmine at an angle she can own, and only goes vertical once the reach test clears. Pressing into range she doesn't have just trains a compensation.
- Deep neck flexors, low load. Strong evidence that low-load craniocervical flexion improves head and neck posture. It's a nod, not a neck curl — loading it heavy recruits the muscles already overworking.
- Pull-biased. Roughly 3:1 pull to push early, easing to 2:1. Only while the balance is being restored.
Honest caveats
- Posture and pain are only loosely related. There's no consensus that posture causes musculoskeletal problems, and the evidence for correcting posture through exercise is genuinely mixed. The reliable wins are strength, range and endurance.
- Twelve weeks minimum. Most postural trials run 4–8 weeks and produce small changes. Judging this at week 3 will only be discouraging.
- Cycle timing. When breasts are heavy and tender, prone work and deep pressing may be uncomfortable. Swap to seated or standing cable versions and drop the load rather than skipping.
- See a physio if symptoms appear. Pins and needles, arm weakness, radiating pain or headaches — stop and get assessed. This assumes the stiffness-only picture you described.
